What is a normal eGFR level?

eGFR estimates how well your kidneys filter waste from blood. It's calculated from creatinine, age, and sex. Above 90 is normal, 60-89 is mildly decreased, 30-59 is moderately decreased, and below 30 is severe. 37 million Americans have chronic kidney disease, and 90% don't know it.

What causes abnormal eGFR levels?

Low eGFR is caused by chronic kidney disease (from diabetes and hypertension — the top two causes), acute kidney injury, NSAIDs overuse, dehydration, urinary obstruction, glomerulonephritis, polycystic kidney disease, and aging. High eGFR (above 120) can occur in early diabetes (hyperfiltration stage) or pregnancy.

When should you see a doctor?

See your doctor if eGFR is below 60 on two tests three months apart — this meets the definition of chronic kidney disease. Also consult if eGFR has dropped more than 5 points in a year, if you have diabetes or hypertension (screen annually), or if you notice foamy urine, swelling, or decreased urine output.

eGFR and longevity

eGFR is the single most important kidney health metric and a powerful predictor of all-cause mortality. Kidney function naturally declines about 1 mL/min/year after age 40, but the rate of decline varies enormously based on metabolic health, blood pressure control, and medication use. The cardiovascular-kidney-metabolic (CKM) syndrome framework recognizes that heart disease, kidney disease, and metabolic dysfunction form a self-reinforcing triad. NSAIDs (ibuprofen, naproxen) are among the most common reversible causes of eGFR decline — chronic use can cause permanent kidney damage. Cystatin C-based eGFR is more accurate than creatinine-based eGFR for individuals with high or low muscle mass.
