What is a normal Folate level?

Folate (vitamin B9) is essential for DNA synthesis, cell division, and red blood cell production. Deficiency causes megaloblastic anemia and elevated homocysteine. It's critical during pregnancy — low folate increases the risk of neural tube defects. Normal is 3-20 ng/mL, with optimal above 5 ng/mL.

What causes abnormal Folate levels?

Low folate is caused by inadequate dietary intake (low in leafy greens, beans, fortified grains), alcohol use (impairs folate absorption and increases excretion), malabsorption (celiac disease, IBD), certain medications (methotrexate, phenytoin, sulfasalazine), pregnancy (increased demand), and MTHFR gene variants (impaired folate metabolism).

When should you see a doctor?

See your doctor if folate is below 3 ng/mL, if you have symptoms of anemia (fatigue, weakness, shortness of breath), if you're planning pregnancy, or if homocysteine is elevated (folate deficiency is a common cause). Your doctor may test RBC folate for a more accurate long-term reading.

Folate and longevity

Folate is a key player in one-carbon metabolism — the biochemical pathway responsible for DNA methylation, DNA synthesis, and homocysteine clearance. MTHFR gene variants (C677T and A1298C), present in 10-40% of the population depending on ethnicity, impair the conversion of folate to its active form (5-MTHF), leading to elevated homocysteine and impaired methylation. For individuals with MTHFR variants, methylfolate (5-MTHF) supplementation is preferred over folic acid, which requires the very enzyme that is impaired. Adequate folate status is essential for epigenetic regulation — proper DNA methylation patterns are a hallmark of biological youth and are disrupted in both aging and cancer.
