What is a normal MCV level?
MCV measures the average size of your red blood cells in femtoliters (fL). Normal range is 80-100 fL. MCV is the most important red cell index for classifying anemias: microcytic (small, <80 fL — typically iron deficiency), normocytic (normal size, 80-100 fL — chronic disease), or macrocytic (large, >100 fL — typically B12/folate deficiency or alcoholism).

What causes abnormal MCV levels?
Low MCV (microcytosis): iron deficiency (most common), thalassemia trait, chronic disease, sideroblastic anemia, lead poisoning. High MCV (macrocytosis): B12 deficiency, folate deficiency, alcoholism, liver disease, hypothyroidism, myelodysplastic syndromes, medications (methotrexate, azathioprine, hydroxyurea), and reticulocytosis (young RBCs are larger).
When should you see a doctor?
See your doctor if MCV is below 70 fL or above 110 fL. Even mild MCV abnormalities warrant investigation with iron studies (low MCV) or B12/folate levels (high MCV). Macrocytosis with neurological symptoms (numbness, balance problems) suggests B12 deficiency requiring urgent treatment.
MCV and longevity
MCV is a powerful diagnostic clue that directs the anemia workup. From a longevity perspective, both iron deficiency and B12 deficiency are highly treatable conditions that impair health when missed. B12 deficiency is particularly insidious — it can cause irreversible neurological damage (subacute combined degeneration) if untreated. MCV begins to rise early in B12 depletion, making it a useful screening signal. Metformin users should monitor MCV as metformin reduces B12 absorption over time.
